To access the Dance • Opera • Theatre (DOT) engagement policy, a DOT Application Form must be filed with Equity.
Your Business Rep must receive the completed Application Form at least 30 days before the start of rehearsals.

What you'll need to complete the form
- Engager information
Nature, mandate, and corporate structure of the organization. - Production information
Name of production, type of production, rehearsal dates, performance dates. - Venue information
Rehearsal and performance location(s). - Security option — Prepayment or Standard Security (see Article 2)
Prepayment means all Artist fees and processing fees are paid to Equity in full in advance.
Standard Security means providing Equity with two weeks’ contractual fees for each Artist, plus additional fees (e.g. transportation for out-of-town Artists, per diems, etc.). - Maximum Box Office Potential (see Article 5)

- Engagement Tier
This determines the applicable minimum fees and is based on the Maximum Box Office Potential. - Complete cast and production team list
Include all Performers, Directors, Stage Managers, Fight Directors, Intimacy Directors, and Choreographers.
Equity will verify whether they are Equity members or members of ACTRA or UDA to determine if they require a DOT contract. - Proposed fee for each Artist
Can be Weekly or Total.

The following documents must be submitted in addition to the Application Form:
- Complete rehearsal and performance schedule
- Engager anti-harassment/Respectful Workplace Policy
Read a template version - Engager health and safety protocols (if applicable)
- Artists outside point of origin details (if applicable)
- Touring production details (if applicable)
